Vision Plan

- Both options cover eye exams and lenses/contacts.
- Both options provide discounts for refractive eye surgery.
- Level of coverage varies by option.
- With either option, you will receive the highest level of coverage when you use in-network providers.
Life and Accidental Death and Dismemberment (AD&D) insurance
- Options offered by Prudential include Supplemental Employee Life, Spouse Life, Child Life, and Employee/Family AD&D Insurance.
- If you waive coverage when you are first eligible, and then later enroll or request additional coverage, you may be required to provide proof of good health status before your election is approved, and pre-existing condition exclusions may apply.
- Beneficiary Designation — It’s important to verify and update your Beneficiary information. Go to our enrollment website and click on “View/Update Beneficiaries” under the “My Benefits Enrollment” section.
